Children’s Blended Case Management
Our Children’s Case Management program provides compassionate, community-based and office-based support to help children and their families confidently navigate life challenges to achieve success. We collaborate with schools, the Philadelphia Department of Human Services, Juvenile Justice Services, Community Behavioral Health (CBH), and other agencies working with the young person to coordinate effective continuum of care.

Children’s Blended Case Management
At our Children’s Case Management program, we work alongside each child and family support to advocate access to services, build everyday life skills, stay connected to essential supports, identify strengths, and set meaningful yet achievable goals. Essential supports may consist of but are not limited to medical care, psychiatric care, community integration, and education. Even though goals are specific and person centered, we maintain a long-term focus on empowering young people to build skills that support self-sufficiency while honoring their individual experiences, diagnoses, and life circumstances.
Each young person and their parental supports partner closely with a dedicated case manager to explore strengths, identify goals, and develop both short-term and long-term plans for successfully community living, maintain stability, and reducing rehospitalization. Together, we create a personalized goal plan that may include but are not limited to:
- Independent Living Skills: hygiene, time management, social skills, food preparation, and dressing
- Health and Medical Management: medications, appointment follow-through,
- Educational and Vocational Support: preparation, networking
- Transportation Planning: navigating public transit
- Community Connection: social integration, engagement in meaningful activities
- Emotional Wellbeing: coping skills, stress management, and personal growth
Enrollment Eligibility
To enroll, individuals must:
- Typical ages range from 5 to 21, but children can be as young as 2 years old. (Young adults over 18 may be eligible if they are still enrolled in high school)
- Have a DSM-5 diagnosis completed within six months of enrollment classifying the diagnosis of a Serious Mental Illness (SMI) determining “medical necessity” for care.
- Must be enrolled in psychiatric care
- Must be a resident of the City of Philadelphia
- Choose to participate voluntarily (cases are reviewed every six months)
Enrollment Information
Referrals may be made by individuals (self-referrals), hospitals, other mental health providers, schools, or may come through the City of Philadelphia’s Department of Behavioral Health (DBH). Referral forms can be found here.
