Adult Case Management 

Our Adult Case Management program provides compassionate, community-based and office-based support to help individuals confidently navigate the resources they need. We work alongside each individual to advocate for access to services, build everyday life skills, and stay connected to essential supports. Essential supports may consist of but are not limited to medical care, psychiatric care, education, and a secondary focus to support co-occurring substance use needs. Even though goals are specific and person centered, we maintain a long-term goal to empower individuals through self-help skills, coping skills, and adaptive skills needed to live successfully in the community, maintain stability, all while reducing rehospitalization, reincarceration, and or relapse.

Each individual partners closely with a dedicated case manager to explore strengths, identify goals, and develop both short-term and long-term plans for daily success and overall wellbeing. Together, we create a personalized goal plan that may include:

  • Independent Living Skills: budgeting, hygiene, time management, food preparation, safety
  • Health and medical management: medications, appointment follow-through
  • Employment support: job searching, preparation, and networking
  • Transportation planning: driver’s license education, navigating public transit
  • Community connection: social communication skills, social integration, and engagement in meaningful activities within the community
  • Emotional wellbeing: coping skills, stress management, and personal growth

 

Enrollment Eligibility

To enroll, individuals must:

  • Be 18 years of age or older
  • Must be enrolled in Psychiatric Care
  • Be a resident of the City of Philadelphia
  • Choose to participate voluntarily (cases are reviewed every six months)
  • Have a DSM-5 diagnosis completed within six months of enrollment classifying the diagnosis of a Serious Mental Illness (SMI) determining “medical necessity” for care.

 

Enrollment Information

Referrals may be made directly by individuals (self-referrals) or may come through the City of Philadelphia’s Department of Behavioral Health (DBH). Referral can be found here.
