The Moderate Care Community Residential Rehabilitation Program (Mod Care) offers an 8-bed residential for adults who are transitioning from higher levels of care to more independent living. The program emphasizes recovery, skill development, and autonomy in a less restrictive environment designed to prepare residents for independent community living.

Moderate Care Community Residential Rehabilitation Program (Mod Care) is a voluntary residential service designed for adults transitioning from maximum care settings who have developed substantial independent living skills but are preparing for full independence. The program provides support and guidance needed to help participants strengthen daily living abilities, maintain stability, and prepare for successful community integration. Mod Care programming allows individuals to maintain independence in a less restrictive setting, while offering a support system to hone skills necessary for self-sufficiency. This gradual transition from a higher level of care to Mod Care, then step down to a more independent setting, promotes long term success of people in recovery.
Mod Care offers a supportive, recovery-oriented environment with program staff throughout the day to provide guidance for independent living and community integration. The program’s philosophy is rooted in the belief that recovery is a journey of healing and empowerment. Our Mod Care offers program benefits such as but are not limited to:

Enrollment Eligibility
- For individuals 18 years of age and over
- Diagnosed with a Serious Mental Illness (SMI) according to the DSM-V
- Average stay about two years
- Actively seeking housing
Enrollment Information
Referrals can be made by local hospitals, boarding homes, mental health residential providers, case managers, self-referrals, or may come through the City of Philadelphia’s Department of Behavioral Health (DBH). Referral can be found here.
