Adult Blended Case Manager (Full-Time)

Role and Responsibilities

Provide an array of extensive case management services to approximately 24-29 adults who have met the criteria for Blended Case Management services. The services provided will include, but shall not be limited to:

 

Specific duties include but are not limited to:

 

  • Assure all consumers on the assigned caseload receive a functional assessment to identify skill deficits and strengths to establish treatment goals.
  • Develop and implement a written service plan for each consumer assigned to the caseload.
  • Identify and connect the consumers to appropriate clinical and community resources necessary to meet the needs identified in their functional assessments.
  • Update and maintain all case management records.
  • Provide services that diffuse or resolve crises in order to prevent unnecessary use of emergency rooms, in-patient services and mental health court.
  • Provide 24-hour response to consumers in crisis, including wearing a beeper or cell phone.
  • Participate in interagency team meetings.
  • Connect the consumer to behavioral health and social service agencies to assure there are no gaps in service.
  • Make home, hospital and community based visits at a minimum of every 14 days (or more often as determined by the consumer’s level of care).
  • Other responsibilities as designated by supervisor.

Qualifications and Education Requirements

Bachelor’s Degree in any of the social sciences and two (2) years experience in public or private human services with one year of direct client contact required. Working knowledge of Crisis Intervention, the Philadelphia Behavioral Health and Prison Systems, Community Resources (i.e. Department of Public Welfare, Educational Law, Office of Social Security and Disabilities and Mental Health Advocacy Programs) required.

BRIEF OUTLINE OF POSITION:

 

  • Perform repairs, including electrical, carpentry, and diagnosis and trouble shoot HVAC, plumbing etc.

Pediatric Van Driver

Brief Outline of Position:

  1. Pick up assigned consumers from their homes, transport them to their scheduled
    programs, and return them to their homes after the end of the program. Assist
    consumer on and off vehicle.
  2. Provide transportation to and from medical or behavioral health facilities for all
    consumers assigned by Logisticare, Inc. per contract requirements.
  3. Complete accurate records concerning the transportation of all consumers
    transported, including arrival and departure times and immediate reporting if a
    client is unavailable for pickup or there is a problem with the drop off. Collect
    parent signatures for minor children.
  4. Provide transportation for consumers and staff for scheduled trips; Vehicle is not
    to be utilized for personal use.
  5. Ensure that the assigned vehicle is in proper operating condition by daily
    inspections and reporting of vehicle condition to the Transportation Manager,
    who will then schedule maintenance and repairs for the vehicle.
  6. Ensure that assigned vehicle is properly garaged at the completion of each tour
    of duty, as well as when the vehicle is not in use for any extended period of time.
    Driver must radio on/off unassigned time such as lunch and break time.
  7. Wear and maintain the company-issued uniforms; be professional and courteous
    at all times.
  8. Driver will be responsible for all paperwork, including but not limited to

Role and Responsibilities

The Substance Abuse Technician (SAT) is responsible for supervising the day to day activities of the clients who are in treatment at the River’s Bend Addiction Treatment Program.

  • Escort clients to all meals.
  • Run 12 Step Education Groups.

Brief outline of position:

Duties and responsibilities:

Complete Program Specialist responsibilities in an adult training facility for people with intellectual disabilities.  Be responsible for a case load of up to 30 adults.

  • Assure compliance of 55 PA Code Chapters 51 and 2380 regulations
  • Complete required documentation including but not limited to monthly reports, quarterly reports, annual assessments, and incident reports and maintain individual records for the program as per regulations.
  • Communicate with families, team members, agencies and community members on behalf of the individual in order to meet his/her needs and regulatory requirements.
  • Participate in the development of the ISP including annual updates and revisions providing content input for accuracy and reporting content discrepancy to the Support Coordinator (SC)

Security Guard (Contracted)

Responsible for maintaining security at assigned CATCH sites with the primary goal of safety for clients, staff and facilities.

  1. Conducts scheduled and unscheduled patrols of the inside of the assigned buildings and the outside perimeters including the fenced-in areas;
  2. Establishes a liaison with the site supervisors/program coordinators to assist in security needs;
  3. Completes a patrol log during tour of duty indicating all security activities conducted;
  4. Provides a periodic presence of security at company parking lots;
  5. Conducts preliminary investigations of unusual incidents and submits written reports of same;
  6. As assigned, conducts in- depth investigations, coordinates investigations with supervisor and submits thorough written reports;
  7. Responds to fire alarms at assigned sites, ensuring that all occupants leave the building and notifies Facilities personnel on call of alarm;

  4. Collaborates with Psychiatric Rehabilitation Counselors to assist PIR in identifying goals and developing personal recovery plans.
  5. Coaches PIR on how to develop and practice skills to achieve goals outlined in their recovery plan and for maintaining personal wellness. Teaches self-advocacy skills.
  6. Identifies community resources as well as other supports, and assists individuals in connecting with resources/supports.
  7. Participates in team meetings and promotes a recovery perspective.
  8. Keeps team and supervisor informed of problematic issues, obstacles, and accomplishments experienced by individuals in working toward their respective recovery goals.
