Anna’s House - Personal Care Home (PCH)
Anna’s House PCH is a 16-bed residential program that serves adults experiencing chronic mental illness and homelessness. The program provides stable housing, coordinated care, and individualized support to promote recovery, personal growth, and successful community reintegration.

Anna’s House: Personal Care Home
Anna’s House is a 16-bed residential program designed to provide comprehensive, recovery-oriented support for adults experiencing a Serious Mental Illness (SMI) and homelessness. The program offers a structured, therapeutic environment where residents receive individualized assistance with housing, healthcare coordination, nutrition, and psychosocial needs to promote stability and community reintegration. With a main program goal to support meaningful recovery and develop independent living skills, individuals work towards obtaining and maintain a fulfilling life within the community.
Residents are encouraged to engage in their recovery at their own pace, supported by a team of staff, comprised of trained Residential Counselors, a Program Manager, on-site Psychiatric and Nursing services who are all committed to providing compassionate and professional care. Anna’s House program benefits include but are not limited to:
- Individualized supports to avoid rehospitalization and/or reincarceration

Assistance with Activities of Daily Living (ADLs)
Health and Wellness Support
Medication monitoring and medication administration
Nutritious Meal Preparation and Dietary Support
- Life Skills and Independence Development
- Individualized Case Coordination
Enrollment Eligibility
- For individuals 18 years of age and over.
- Diagnosed with a Serious Mental Illness (SMI) according to the DSM-V
Older residents typically have concurrent medical issues
- Average stay ranges from a minimal two to three years.
- In need of housing
- Must be able to travel via public transportation and must be self-sufficient.
Enrollment Information
Referrals can be made by local hospitals, boarding homes, mental health residential providers, case managers, self-referrals, or may come through the City of Philadelphia’s Department of Behavioral Health (DBH). Referral can be found here.
