Intensive Community Residential Rehabilitation Program (CRR Max)

The Intensive Community Residential Rehabilitation Program (CRR Max) is a highly structured, community-based service designed to support adults with Significant Mental Illness (SMI) needs in achieving recovery, stability, and successful community reintegration. The program primarily serves individuals transitioning from extended periods of incarceration or institutionalization, providing the structure and support necessary to rebuild independent living skills and confidence.

Participants include adults of varying ages ranging from young adults requiring intensive supports to older adults with SMI. For many, the CRR Max program represents the first step toward renewed community engagement and personal recovery. We are committed to empowering each individual through program benefits but are not limited to:

  • Designing an Individualized Rehabilitation and Recovery Plan
  • Independent Living Skill Development
  • Supports to Establish or Regain Community Engagement and Social Integration
  • Designed for Person-Centered Care

Enrollment Eligibility

  • For individuals 18 years of age and over
  • Average stay is minimum of two years
  • Diagnosed with a Serious Mental Illness (SMI) according to the DSM-V

Must be appropriate for temporary residential support and rehabilitation services.
Individuals seeking to stabilize, recover, and prepare for independent or less restrictive living environments.

 

Enrollment Information

Referrals can be made by local hospitals, boarding homes, mental health residential providers, case managers, self-referrals, or may come through the City of Philadelphia’s Department of Behavioral Health (DBH). Referral can be found here.
