Moderate Care CRR is an 8 bed voluntary program that provides residential services for eight (8) persons who have graduated from a maximum care living situation.
Moderate Care CRR is an 8 bed voluntary program that provides residential services for eight (8) persons who have graduated from a maximum care living situation.
Participants in this program have substantial independent living skills but are not yet ready to move into permanent housing. In an apartment setting, people live as independently as possible with staff support and/or assistance. Residents take their own medications, purchase food, cook their own meals and take care of personal needs. The program focuses on helping people live productively in this less restrictive setting.
Program Summary:
- Age Group – 18 and over.
- Who can refer – All of our referrals are through the Transitions, Integration and Partnerships (TIP) program of the City of Philadelphia Department of Behavioral Health.
- Eligibility – Residents seeking Housing who have a controlled mental health DSM-V Diagnosis and are in need of Housing. Must be able to travel via public transportation and must be self-sufficient
- Length of Treatment – Minimum of two years
