Lehigh House: Long Term Structured Residence (LTSR) and Partial Hospitalization Program (PHP)

Lehigh House is a 16-bed integrated residential program that provides comprehensive treatment and rehabilitation for adults with chronic mental and behavioral health challenges. The program holds dual licensure as both a Long-Term Structured Residence (LTSR) and a Partial Hospitalization Program (PHP)—offering a unique continuum of care that bridges the transition between inpatient hospitalization and independent community living.

Lehigh House is dedicated to fostering a trauma-informed, therapeutically rich environment where individuals are supported in achieving their recovery goals. The multidisciplinary treatment team comprising of psychiatric, clinical, and rehabilitative professionals all collaborate to ensure each resident’s care plan is person-centered, strength-based, and informed by current best practices in behavioral health care. Each personalized care plan may include but is not limited to:

Independent living skill development
Integrated treatment model combining LTSR and PHP services
Comprehensive, person-centered care planning and personal empowerment
Evidence-based and trauma-informed therapeutic services and intervention practices
On-site psychiatric and medical care along with a collaborative, multidisciplinary treatment team
Skill development for community integration
Support for transition from inpatient settings to community-based living through community engagement
Safe, secure, and recovery-oriented residential environment

 

Enrollment Eligibility

  • Adults with a persistent Serious Mental Illness (SMI) who have reached maximum benefit from their referring environment but continue to require structured, community-based treatment and support.
  • Length of Treatment varies based on the individual’s clinical needs and recovery progress
  • Must be 21 years or older

 

Enrollment Information

Referrals are accepted from extended inpatient facilities and acute psychiatric hospitals within the Community Behavioral Health (CBH) network. Referrals may come through the City of Philadelphia’s Department of Behavioral Health (DBH). Referral can be found here.
